We are committed to providing quality care and helping you maximize your insurance benefits. Payment for uncovered services and out-of-pocket costs is due at the time of service. We accept cash, checks, debit cards, Visa, MasterCard, and Discover (returned checks incur a $25 fee). We file claims only for participating plans with verified benefits. Please remember your insurance contract is between you, your employer, and the carrier—not our office. Some services may not be covered or may have limits, and these costs are your responsibility. While we file claims as a courtesy, all charges remain your responsibility from the date of service. If you face financial difficulties, please contact us for assistance.
As a courtesy, we verify insurance eligibility before your visit, but this is not a guarantee of coverage. Patients are responsible for charges not covered by insurance, so we encourage you to understand your plan and its benefits. Bring your insurance card, photo ID, and payment for your copay/co-insurance; without your card, full payment is due at the visit. We do not file claims for indemnity plans.
